Welcome to Lakeshore Transportation LLC. These Terms of Service ("Terms") govern your use of our website at lakeshoretransportwi.com and our non-emergency medical transportation (NEMT) services. By accessing our website, booking a ride, or using our services, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use our website or services.

1. Services Overview

Lakeshore Transportation LLC provides non-emergency medical transportation for seniors, individuals with disabilities, and others in need of wheelchair-accessible or assisted transportation in Sheboygan County, Wisconsin, and surrounding areas. Our services include transportation to medical appointments, therapy sessions, dialysis treatments, day programs, personal errands, and social outings.

We serve private pay clients as well as individuals whose transportation is coordinated through state-funded programs including IRIS, My Choice Wisconsin, Community Care, and Inclusa.

2. Ride Booking and Confirmation

Rides may be requested by phone at (920) 980-2080, by email at [email protected], or through the online booking form on our website.

Submitting a ride request through our website does not guarantee a ride. All ride requests are subject to availability, scheduling, and confirmation by our dispatch team. A ride is only confirmed once our team contacts you to verify the details.

You are responsible for providing accurate information when booking a ride, including the rider's name, pick-up and drop-off addresses, date, time, wheelchair requirements, and estimated rider weight. Inaccurate information may result in delays, cancellation, or inability to provide safe transportation.

3. Cancellation and No-Show Policy

We understand that plans change. We ask that you provide as much advance notice as possible if you need to cancel or reschedule a ride. Cancellations should be communicated by calling our office at (920) 980-2080.

Repeated no-shows or last-minute cancellations may result in service restrictions. For rides coordinated through state-funded programs, the applicable program's cancellation policies may also apply.

4. Rider Responsibilities

When using our transportation services, riders and their representatives agree to:

  • Be ready for pick-up at the scheduled time
  • Provide accurate health and mobility information relevant to safe transport
  • Treat drivers and staff with courtesy and respect
  • Follow all vehicle safety procedures, including the use of seat belts and wheelchair securement systems
  • Refrain from smoking, using illegal substances, or engaging in disruptive behavior in the vehicle
  • Notify us of any changes to the scheduled ride as soon as possible

5. Our Responsibilities

Lakeshore Transportation LLC is committed to providing safe, reliable, and professional transportation. We will:

  • Maintain our vehicles in safe and clean operating condition
  • Employ trained, courteous drivers who are equipped to assist riders with mobility needs
  • Make reasonable efforts to arrive on time for scheduled pick-ups
  • Treat all riders with dignity and respect in accordance with our People First philosophy

6. Limitation of Liability

Lakeshore Transportation LLC provides non-emergency medical transportation services only. We are not a medical provider and do not provide medical care, advice, or supervision during transport.

To the fullest extent permitted by law, Lakeshore Transportation LLC, its owners, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services or website, including but not limited to delays, missed appointments, or service interruptions due to weather, traffic, mechanical issues, or other circumstances beyond our reasonable control.

Our total liability for any claim arising out of or related to our services shall not exceed the amount paid by you for the specific ride giving rise to the claim.

7. Payment

Payment terms vary depending on your arrangement with us. We accept private pay clients and work with state-funded programs. If you are a private pay client, payment is due at the time of service or as otherwise agreed. If your rides are funded through a state program, payment and billing will be handled in accordance with that program's procedures.

8. Website Use

You agree to use our website only for lawful purposes and in a manner that does not infringe on the rights of others or restrict their use of the website. You may not:

  • Use the website in any way that could damage, disable, or impair it
  • Attempt to gain unauthorized access to any portion of the website or its systems
  • Submit false, misleading, or fraudulent ride requests
  • Use the website to transmit harmful software or malicious code

9. Intellectual Property

All content on our website, including text, images, logos, and design elements, is the property of Lakeshore Transportation LLC or its licensors and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from our website content without our prior written permission.

10. SMS/Text Messaging Terms

If you opt in to receive SMS messages from Lakeshore Transportation, you consent to receive text messages regarding ride confirmations, scheduling updates, and occasional marketing communications. Message frequency varies. Standard message and data rates may apply.

You may opt out at any time by replying STOP. Reply HELP for assistance. Consent to receive text messages is not required as a condition of service.

11. Third-Party Links

Our website may contain links to third-party websites. We are not responsible for the content, accuracy, or practices of any third-party sites. Visiting external links is at your own risk.

12. Indemnification

You agree to indemnify and hold harmless Lakeshore Transportation LLC, its owners, employees, and agents from any claims, damages, losses, liabilities, and expenses (including reasonable attorney's fees) arising out of or related to your use of our services or website, your violation of these Terms, or your violation of any rights of a third party.

13. Governing Law
